1) Yes, you neet to fill in only one form - for yourself.
2) You don't mention your dependandts in the HSMP application form. This can be done in the covering letter. Dependants issue will be brought up at the Entry Clearance stage, after you get your HSMP approval letter.

Well, to claim 10 points for the skilled partner one needs to prove that they've been in a relationship for at least 2 years, and it doesn't matter if they are married or not. According to the caseworkers' internal guidelines, a birth certificate of a child can be considered as evidence of marriage ONLY if it has your address written in it.

it's an approval!!!

I was told over the phone that my application was approved!
It is my second application. In the first one I claimed 80 points and it was rejected; the HSMP Team decided I didn’t provide enough evidence for my work experience. Now I claimed only 65 points (under 28).
