Online MN1, should I ask doctor for personal details

Online mn1 form is asking for referee details like passport, address etc..

Now how should I proceed should I ask doctor to provide me these details or somehow I can skip them then after submitting and printing application doctor write details themselves using pen

Yes, it sucks, you need to ask for the personal details. I know, completely unreasonable to expect us to ask people we only interact with professionally for their private details. But that's what they do.

No you do not need a 'letter'. Just what the form asks for.

I just phoned them, they said because my daughter is only few weeks old, doctor can't provide reference and needs to know the applicant for 6 months at least.

Now I am not sure who else can I get reference from :(

Also if I manage to get details, do I need to get there signatures after submitting application as well ?

Yes, 1st you need to fill all the details & once you submit the application form, you will get referee forms which needs to be signed by the referee after you attach your photograph to it. Its ridiculous to ask for all the passport details etc from a professional referee, but we have to do it in order to complete the application form.

My doctor is part time, they said they will take a week to give me details and now I will need another week for signature, that's awful.

My child are only 3 years and one 2 weeks old.

Can i ask anyone else ? Ideas ? please ?
