Settled person Spouse and kid outside UK..urgent

Gurus,
I am settled in UK, now am applying visa for my wife and child in India, i have met the financial requirement of £22,500 per annum.
1 ) can i submit visa for my child in the same application form of my wife, or do we need to make a separate application for my child The VAF4A online form
2) could you please let me know the exact fees for spouse and child ...bit confused from outside uk ( i heard from one of my friend news fee structure from october? )
