This could be the reason but i have read in the other forum that if you book your appointment beofre April 6 even if your appointment if after April 6 you will be charged acc to old fee

2. I was only out of the UK for a couple of weeks each year on Annual Leave - since I was not outside UK for long at all each year, so do I still need get a Employer Absence from UK letter please?

3. When booking online, is asks What Form where I choose SET(O) and then it asks me Which category of leave are you applying for? - What do I choose here - I am not applying for Tier 1 (G) as I am applying from T1 (G) to ILR, so should I choose 'Other Main Applicant'? - but I am applying with 2 dependants.

You should choose 'Tier 1 General' as this is your current visa status.
