PBS dependent PEO application form

Hi

We have a peo application soon. When I login to the peo website and click on download form, it’s only a 2 page pre-filled form which doesn’t seem right?

Which form should I print out and fill in? Is this the correct one? https://assets.publishing.service.gov.u ... -04-18.pdf

The one in the link above is correct paper form.
