ESTA & Transit Visa

Any UK citizen travelled to US recently?
Is it on arrival visa given in US airport of entry or we need ESTA in advance?
If we need ESTA , is this online process or we need to submit our passports and need to get stamping ?
whats the usual processing timelines & cost for same pls?

2) Indian passport holder (with US visitor visa) need a transit visa if we are travelling via Lisbon(Portugal)

Online info is bit vague hence asking here from recent travelers. Thanks

1) ESTA is an online only application BEFORE you travel to the US, if your country qualifies for ESTA "visa waiver" (UK citizen is ok) - https://esta.cbp.dhs.gov/. It's not a visa - US entry officer though has the final say for entry clearance.
2) don't know.

You shouldn't need a visa to transit through Portugal to the USA.

Just curious what airline this might be?

Not if transiting via Portugal or any of the other Schengen Zone states except Germany, France, Spain and the Czech Republic (the latter four require Airport Transit Visas for Indian passport holders).
