There's only one form to fill out for surrendering your passport. Yes, you need your British passport before applying. When you surrender Indian passport at VFS; along with SC-Surrender Certificate, do you also receive the cancelled passport back (with cancelled stamp)?
This can be useful to prove (in future if needed) that you held Indian passport in the past.

Obviously this is for passports, but extends to all consular, oci related jurisdicton as well. And how do embassy/consulates ascertain the applications jurisdiction? Address proof.
Inconsistent approaches by the individual employees at HCI may be giving wrong impression!
