French wife applying for OCI

Hi All,

I am a British Passport holder living in UK with my French wife. My BP was acquired last year, along with OCI (after surrendering my Indian passport). My wife (married for 2+ years) is in the process of applying for an OCI & has been asked to provide the following -

Applicant should also carry a consent letter written by the spouse (of Indian Origin) providing consent for the applicant to be able to obtain an OCI based on his/her Indian Origin. Letter needs to be addressed to the High Commission of India, Consulate General of India, Birmingham or Edinburgh as the case may be.

I was wondering if anyone has provided this recently & has a template? I want to be sure of what information needs to be included.
