''Payslips covering any period of salaried employment (current and previous) in the period of 12 months prior to the date of application''

Am I required to provide 12 months payslips or six months?

If i apply using premium service, what will be used as the day I submitted application, will it be when I submitted online or when am to go for interview.

Our income depends on how it is calculated. Knowing whether the employment is Category A or B and whether salaried or non salaried will decide how much we earn individually. We have both worked for the same company for more than 6 months.
