Also, for the NHS registration document which is to be submitted for my over 18 dependants, is a letter from my surgery, indicating the date of my registration as 2018, enough? The surgery letter does not have ‘NHS’ written on it.

Yes, I applied using separate applications for my over 18 dependants but they only asked for the record of absences from my spouse that is why I asked.
Yes, I have proof that they don’t live independantly, thank you!

Also is it normal that on my wife’s application they’ve asked this question: “ does the period of stay on which this application was based include any leave granted since 11 Jan 2018”? I firmly believe that as per rule/ guidelines on the subject ; there is no need to do it again your old IELT B1 qualification should suffice however expert opinion of moderator are requested please
