Infact the ILR approval letter is actually suffice to apply for dependent and you dont need to wait for card actually. Thats how I did after getting ILR on 8th Nov.. It is possible to do in same day as well but it ll not be a same appointment and should be booked bit late for next appointment ...

No idea but I am just going with standard documents for Contractor

e.g.accountant letter for income/paylisp/dividends voucher/interim account/invoice etc
