You are over-thinking this. They are not interested in your medical history, but in whether you have any unpaid debts to the NHS for medical treatment, which is a general ground for refusal. Assuming the university has all the above information, it is possible that the issue is the 5-month absence from the UK in 2016. It depends on the circumstances of this.
