When you are applying online and select adult/ child, you will get the information as to which form you need to fill. I got the information from there. The form for the child is very lame, but it is what it is.

Why do you insist on extending the Tier 2 ICT Visa? After you were granted ILR, the ICT visa is no more. No chance to extend it at all. I am not expert, but whoever advised you at UKBA was wrong.
