Employed or Self Employed to be mentioned in Tier1 Extension

I am an IT contractor and I own a registered company in UK. I am the director and 100 % share owner. I take Salary + Dividends. My Accountant confirmed I should be treated as Employed even though I am a director.My accountant says I Pay class 1 NIC Insurance to HMRC. Accountant agrees to give all the salary + dividend break ups with Payee/NIC/VAT Paid during the assessment period for Tier 1 extension.

What should I mention in HMRC application as employed or self employed ? Does it makes any problem while applying for extension
