PEO Checklist

The checklist provided for the PEO appointment has the first one as

- Print and sign the official form and bring it to your appointment. To do this, click Continue below.

But when I clicked continue, it reverted back to the previous page which is the appointment listing. Is there anything I am missing? What is the "Official Form"?
