Sure Name and Payslips

Hello All,
My name is Tareq Muhammad Mahmoud Ali
My payslips issue with Name Tareq Mahmoud, not Tareq Ali, i.e. they didn't use my last name they use one of my middle names. I contact our HR department to fix it, they said they can't fix it
"I cannot change your name on the payslips retrospectively, however I can going forward."
Do you think this may causes a problem.
Secondly, I lost one of my payslips

Note: in my bank stmt, my employer name is included.

Another Question:
I'm issuing a letter from my university, to proof my language skills
"An original letter from the awarding body on the official letter headed aper. The letter should confirm your name, the qualification obtained, the
date of the award and whether or not the course was taught in English."

The problem that they don't include contact information on that letter. do you think this letter will be refused? I can include a contact information for the graduated students department manager.
