Queries for Tier 1 (General) Form - Dependent

Hi,

I am applying for the second extension for Tier 1 (General). I have few queries about the form mainly dependent's form:

E7: Do I need to mention his work Telephone as he is working as well. Do I need to provide any extra documentation for his job?
Section F: We don't have previous letters with us. Shall we leave that blank?
K 1-13 : Last entry: Do we need to give information about 1st extension of visa or about the first ever visa (of Tier 1 General Dependent). I guess they are asking for the latest travel and latest permit as mentioned in help text but just wanted to clear the confusion.
K 18: Previous entry refusals: Spouse first visa was refused because we didn't provide the marriage pictures as we were living together in Pakistan after marriage and had one child. We went for appeal and got the visa stamped. Do we have to say "No" in question aor say "Yes" and give whole detail of refusal. We don't have refusal letter reference number but we do know the case precisely.
Section M : Maintenance: I, as main applicant supporting my whole family. My daughter and spouse doesn't have the bank statements. But M9 and M10 is asking that (dependent) has sufficient funds. Can I mention this in cover letter? Moreover he is also working but we are maintaining only one savings bank account which is my name.
N12: Sole Surviving Parents? We both are working and upbringing family. shall I say No to this question. Do I need to provide my husband's work evidences? like P60 or salary slips or letter from employer?


For the main Applicant Form:
Question B14 is only asking 1 extra address in last 5 years whereas we have been on three addresses in last 5 years. Shall I mention that as well. They are clearly asking this information on dependent application form but not for the main applicant.
Any evidences do we need to provide for previous addresses?

Can please someone help me in answering these questions.

Here I have some answers to my own question. I hope it may help to others members.
E7: I didn't provide the work phone number of dependent as it is only necessary for the main applicant.
K1-13: I gave information the last time we traveled to UK.
K18: I gave all the information when and why visa was refused at first stage and attach the letter which states that we won the appeal.
Section M: I mentioned about maintenance in cover letter.
N12: I have mentioned that we both parent are responsible for upbringing of our children both morally and financially.
I have attached last 5 years addresses anyways with main applicant form. (I want to be on the safer side). I didn't provide any evidences for that as they have our last extension on previous address.
