Dependent form - please help..

Dear All,
I am right now filling the application form for my dependent. On section B5 - Main Applicants date of Birth under SECTION 2 - DEPENDANT’S DETAILS.

I am bit confused.

Which date i will enter here -- Main Applicant (me) date of birth

OR

Main applicant (dependent- my wife)date of birth


Please help at your earliest.
