Enquiries about Tier 1

Hi,
I have a few more queries in Tier 1 General application form.
1) Post (City) - which is below the country where i obtained the visa or entry clearance. Should i put Chennai or Bangalore (where I posted the application).
2) In contact details.... do i need to provide the address of the bank and also the telephone phone details.....
