Tier1 Dependent Documents Checklist & Covering Letter

Hi,

I am currently on Tier1 General Visa, and want to apply for dependant visa for my husband. My husband was on UK work permit so far, but his work permit will expire in few months. He is in India currently.

1. Can anyone help me with a documents checklist for this tier1 dependant application?
2. Can someone provide covering letter template please?
