Chartered accountant-- total income

Do we have any specified format to get the total income earned in the last 12 months and quoting the pound figure ? I planning to have a document from my Chartered accountant saying that the following X individual has earned XXX INR which is equivalent of YYY pounds as per the Conversion rate of 1 Pound equal to 77 INR. Please let me know if I need to modify in any way.
