Dependant application form help

Hi,

I am applying for my family with my tier 1 application. I would like to get clarification for the following questions.

Dependant form
-------------------
1. A2.2 Contact name in the UK if different to that of the dependant. - What should I put here? If the dependant is minor, should I put my name?


Main form
-----------
1. D5. Are either the applicant’s current and/or any one of his/her previous passport(s) or travel document(s) that he/she has used to travel to, and remain in, the United Kingdom which shows his/her current leave unavailable? - Can you please explain this?

2. Should I attach all (mine as well as dependants) photographs with my application?

3. In the summary section I have to provide numbers of each documents. In the main application should I give total number of passports including the dependants?
