Tier 1 Immigration history

Hi,
I am applying for Tier 1 extension on 5th march. can anybody help me answering the following question?

D1. Did the applicant obtain entry clearance or a visa before entering UK?
Yes/No
is it the first entry clearance which i got when i came to uk in 2003 ?
i got my Tier 1 visa when i was in UK. should i select yes or no?

On the dependent form
L3. Reference number of the main applicant’s most recent application to us:

I dont have the ref number now cause i lost home office letter when i got my tier 1 visa. where can i get this from?

Its the last EC you had before entering UK

reference number is an optional field so you can skip it
