Help booking Premium appoitnment

Hi,

Have few queries regarding booking appointment for Tier 2 General

1. When trying to book premium service appointment. I could not find Tier 2 PBS general . there is an option of Tier 2 (Sponsored Skilled Worker)?
2. My current visa expires on the 28th of Jan 2013. When Can I start looking for the dates for the appointment.
