Cover Letter from myself

Hi,
i'm writing the cover letter for my Uk tourist visa application containing the following information:
1) my name and personal informations;
2) dates and points of arrival and departure in and from UK;
3) itinerary in UK.

Is there someone who can give me advise about the structure of this letter?
Is there a sample letter i can follow?
