Delayed UK Visa Processing from New York Post

My UK visitor application is still pending decision but my passport has been returned without a stamp or decision letter last week along with husband's passport who got his visa. We both applied together and both passports have been returned in one envelope. Did anyone run into this situation before? What does this mean? VFS rep keeps saying that my application is still pending
