Children's Endorsement On Mother's Passport

My friend is on a work permit and the visa is stamped on wife's passport as well.

His children were born here and were endorsed by Pakistan embassy here on the mother's passport.

Now he has gone back home and the immigration in Pakistan are not letting the children come back. Apparently they haven't got the HO's endorsement for children.

Anyone else heard of such a case?

The only recommendation I made to him is to apply for the children's visa. But it seems so strange that they were able to travel without a visa in the first place (even though it would seem logical for Heathrow to not "check" anything on exit, its the entry that really matters)
