Question about green card

Hi, i have a question about green card system.
I saw that you need a job offer to be able to apply for Green card: this is possible only for the peaple that are already in Irland?
I mean: no one outside Ireland will be able to get a job offer from an Irish recruiter even if he has all the skills required in the shortage occupation list?
