How many letter you received before approval letter

Hey just wondering if all of you got 2 letters before the approval letter? A friend of mine applied a year ago and got reference number after 4 weeks and then after about 11 months he got approval letter and was asked to submit 950 euros. He never got the second stage letter. Did it happen to anyone else too? Getting approval letter without getting the second stage letter.
