settlement question

Hi,
New to this board and have been on the Entrepreneur visa for nearly 5 years now. I'm looking at the settlement form and am confused about something in Section 11, specifically 11M. Where can I get clarification? The Home Office website says to contact a solicitor or advisor for questions on filling in the form. Any recommendations?

The question is very simple. I've been on the Entrepreneur visa the entire time, but the way 11M is worded, it's not clear to me if I have to send in documents about National insurance or payments to HMRC.
