I am going to apply for my Tier 1 Entrepreneur Visa extension soon. Just want to confirm the forms to be used.

1. For my application, I understand that I should use the form in:

2. However, for my dependant, should I use this form:
https://www.gov.uk/government/publicati ... -dependant

For reference number in B11 of the form, should I use the Tier 1 Entrepreneur visa application reference number?
