Tier 2 employer sponsored .. MUST * requried documents

I would like to know that when applying for tier2. . apart from employer certificate of sponsorship CoS

do we still need to attach

Ielts or toefel certificates (all the education has been in English throughout)
bank statements .
any other documents etc that are must required.

also his sponsorship is dated about 45 days back. is that ok ?

I am also in the same process of filing.

I went through ukba documents and it says

1,You need Cos assigned by your sponsor

2, 90 days balance of £900 or your sponsor's approval on COS form to say that they will take care of you maintenance funds.

3, english language score. If you are graduated from UK universities you will get these points straight away by submitting your UK degree certificate.

4. the most important thing is your salary and job description or SOC should match and above the minimum requirements.

AND needs to be NQF 6 (or NQF 4 Creative Sector).
