Tier 2 Priority application inclusion form

hi there,

can any body tell me what to write under Name of authorised person, Contact number and Email on sheet which need to be sent by email for inclusion on priority service for Tier 2 ? (I am applying myself/not through any representative) I am asking because I think i need to enter my contact and email .

on the form

the questions re. authorised official, email address and phone number refer to the representative. If you don't have a representative leave these blank.

The form has to be sent as attachment to an email. In the email itself iinclude your contact details

This is my opinion, I don't have direct experience using this form
