Tier 2 (Gen) Extension

I have to apply for Tier 2 (Extension) for me and my dependent, however I am not sure if I am following this properly. Hopefully someone can guide me

1) I am filling an online application using the below link

https://www.gov.uk/tier-2-general/extend-your-visa and selecting the option 'Apply Online as a Family Group' as I have to apply for my Dependent along with me and selecting 'Tier 2 Migrant online application'

Is it the right place?

2) When I open the form it says 'Tier 2 (General) April 2013' -

3) I have to apply for my extension and my COS has been assigned to me for 3 years period, however in the form I do not have the option to enter the number of years I am applying for, also it does not have a question if the application is for Tier 2 General Extension. Please see below

Am I doing something wrong?

4) Under Dependent Maintenance Section, do I have to show the maintenance as the application is for extension and I have already satisfied the requirement during my Tier 2 General application. Which Option do I select from below

It appears my dependent needs to have money in her account?
