Application Form Changes

Hi

I am in process of recently switching to new employer under tier 2 general and when i booked a premium one day appointment before 2 years, I had also filled a big application form and take it with me but now I couldn't find any application form to fill it and take with me to the appointment.

Have they abolished it ( no more big application) - just the small application form we fill when booking premium one day service.

Could anyone assist with this please.

It is ONLY an online application form. There is no longer a paper form.
