Tier 2 General Extension- Same Employer & SOC

I am due to apply for my Tier 2 extension. Below are key highlights of current visa

Visa issued 13-March-2016
Visa Expiry 28-Feb-2019
Dependent: Wife with BRP ( 1 child recently born and due to receive Pakistani passport around mid Nov-18 so child is not on any visa)

Questions:
1) What is the earliest date my employer can assign a Ucos (allocation is already granted) ?
2) What should be the start date on Cos and end date?
3) How early can I apply? I believe it’s 3 months before start date on Cos. Trying to avoid the increase in IHS so please specifiy the exact date an application can be submitted at earliest?
4) My Tier 2 visa was initially granted for 3 years from 13-march-2016, what should be my minimum salary for SOC 3534? Would it be 30K or experienced salary of my code which is 27K as my first cos was assigned before 24-Nov-2016.
5) Am i Ok to include new born child as dependent with my current tier 2 extension? She does not have any BRP.
6) Had some tax credit this year due to change in tax code as wife never worked in UK. I was told to get tax code amended and have some tax credit. Does this effect my current tier 2 extension or ILR in future in any way?
7) I am sure we don’t need to prove the continuous relationship which we had to when I initially applied my wife’s visa as dependent, but should bills etc be included which are under her name to prove the same address?
