Skilled Worker Dependent Visa Extension

Hi Guys,
I was given ILR based on Tier 2 (Gen) Visa.
My wife applied for extension as a dependent before I got ILR but got her leave to remain after I was granted ILR. That leave is about to expire.
We are planning on applying for another extension as a Tier 2 (now Skilled Worker) dependent, as she does not yet qualify for ILR. If we switched over to Spouse Visa, we would lose the residence period and having had to start all over again.
On the form, they are asking:
1. Have you ever had medical treatment in UK (include visits to GP).... Our Answer -Yes
2. Then it asks if you were asked to pay for that treatment.(This does not include the IHS charge) Yes / No.

If we say No, as she paid the IHS charge in her previous extension and did not have to pay for treatment, it asks for Start & End Dates of those treatments.
We also had a baby so there would be maternity visits to the hospital as well.
There was once some dental treatment for which she had to pay as well.
Obviously, we do not have a record of exact dates for all of those since she came to the UK.

How should we go about answering this question ?
Tried to contact the current surgery but they were not very helpful as they say that it would take a long time to retrieve the record of appointments and we are running out of time.
