Evidence of relationship

hi, im from india, i need help on this Evidence of relationship for a tier 4 student visa for uk.

i do not have a birth certificate, so the form says i need a Court document naming the student and the legal guardian(s).
could someone please tell me the procedure to get this form? is the form called anything?
im totally clueless, do get it from a lawyer or a notary public or who else...?

please help!
