Now the time for Naturilasation.

1) What do I write in "2.1 Date and place of first arrival in the United Kingdom ."
Shall I write March 2002 when I came on student or
when i came on HSMP, intend to came for long time?

2) Referee Shall I put two of them british and two of them professionals.
ex: First Accountant and second is Solicitor/Barrister.

When you first arrived is fine

You only need one to be a professional of any nationality and one to be British passport holder, you can of course use two British Professionals.

Just the documents as stated in the AN guide. If you've had your passport for the last 5 years you don't need any more documents for residency. You don't really need to send the Life in the UK Test again as you used it for your ILR application. However, some NCS like to see it.

5) After applying I am travelling to india 3 months. once i received letter
approval do I need to call council for delay in ceremony as I am in
india for 2 months.

The council will invite you to book a ceremony which must be done within 3 months.

thanks in advance.

british referee does't have to hold passport for 3 years.
As per lituk tick first one.
Put any occupation you like, they don't bother checking it.
