Was self employed, now employed--- what Docs for BC?

I was self Emplyee until April 2013, now employed and my BC application due on 04/2014
Do I have to submit self assessment statement from HMRC or not as I am not self employed any more.
My ILR application was based on self employment and ILR granted on self employment bases.

Please suggest

Have you read form AN?

Do they ask for your employment or self employment history on the form?
