AN Form TAX office Address or PAYE reference

Hi,

I have very urgent query please. I am applying for naturalization and unfortunately i finished my job on Friday the 5th of Sept 2014. My appointment with NCS is on 11th Sept 2014.

What should i write down in Question 3.5 where its asking for Address of tax office or tax office reference.

Will really appreciate your prompt reply.
