Advice on filling passport from

Sec 3, do i need to write all passport. ive two passport first one is cancelled and got ilr visa on that one.

Sec 9, for kids, do i have to put my name or child name

Sec 9, if signature date errors, how can i correct.

Sec 10, for kids, referee put my name or child name

sec 10, counter signatory can say just friend.
