British Child Passport - both parents EU citizens

Me and my wife are both EU citizens. We have Registration Certificates confirming Permanent Residency which were issued before the child was born.
Is it enough just to include the EEA3 Permanent Residence Documents for the childs passports or we need to prove 5 years of treaty rights to the passport office?

The blue booklet (together with your passport) is enough.

Would I also need to include covering letter? Would they be aware at the passport office about the blue booklet, or I need to point it to them?
What about Grandparents details, are they needed in this case?

anyway they should be aware of it by now
