AN application query - pprevious addresses

While filling in AN form, if you have provided current address and date you moved into, in section 1.16, then in section 1.17 (previous addresses for past 5 years) do you start from your current address again and go backwards or just start from the previous addresses??
If you start from the current address in section 1.17, then what do you enter in the "TO" section .... "to date"? or "date of application i.e., date it is being submitted at NCS" or leave blank?
