I'm currently completing the form AN and wondering about the question "3.5 Address of tax office or tax office reference" on this form.

Should I give the address that is stated on the letter that I receive with my tax code from HM Revenue & Customs or it's referring to something else?

Well I have submitted the Self Assesment statement address (if You a director of a ltd company). If PAYE this should be your tax office address check your p60.
