need help about passport appointment

hi
i need your help.my son 5 y.o.recieved british cert.yesterday.i called passport office to make an appointment.i'm not sure what documents i need to bring with me.who can be professional person (doctor or teacher ok?)to sign his photograph.should i fill the whole app. or maybe some points i leave blank.this is first passport for my son.i've got his birth certificate and british c.do i need to provide EEA id card or old passport which is out of date any way.thank you for your help.i have checked this quide how to fill passport app.but maybe someone done it before and could give me some advice.thank you
