Name at birth in the Form AN

Hi,
My wife was using my surname for all visas and ILR though her passport has her own surname. Now in the application form AN for the below question do I need to give her previous full name with maiden name?
1.7 Name at birth if different from above. (If the names you have given are different or spelt differently from the name
shown on your passport, please explain why on page 13)
