MN1 application queries- Advise needed.

Hi
I got my ILR about 10 days ago so i am applying Naturalisation for my Dtr who was born in UK Under Section 1(3) of MN1 but have few queries.
Section 1
A.1.2. I have correspondence from UKBA but as per the guidance it says it should start with S followed by 7 digits,i havent got such reference except for the BRP card numbers, can i leave it blank ???
B.1.3.While applying for my ILR i did not apply ILR for my dtr she is on Tier 1 dependent which is valid until July 2013.Do i need to put her DOB as she was born in UK because of the above.
Section 2
C.Do i need to fill 2.1,2.2 and 2.3 if i am applying under Section 1(3).If i had to fill do i need to put my dtr's DOB in 2.1 as she is UK born.Question 2.2 -My Dtr always lived in UK except for 80days when she went India so its less than 2 yrs,please advise me on this.
Section 5
Can the priest of our temple who is a British passport holder who knows me and my dtr be a second refree as the health visitor who is an English lady refused to sign by stating that its too complicated.
