Form filling questions

Hello,

I know that some of these questions might sound silly but I think it is better to be safe than sorry... especially with the amount of fees UKBA now charges. So please bear with me.

Q1: I did not pay any PAYE for 6mths in last year, though in this financial year I have started paying PAYE (I am a director of a company est in 2010) - Do I need to attach a self assessment for last financial year?

Q2: In section1.7 it asks for any other name that you've used in the past - Does this mean in my wife's application I need to write her full maiden name (she hasn't changed her first name)

Q3: My wife was in India for 8 mths frm Aug-08 to Mar-09. - What address should I list in address history - the same as my UK address at that time or the Indian address?

Q4: Do I need to write a covering letter?
