Filling in the Form UKM - advice please?

My mother is English and migrated to NZ in the 1970's and I was born in 1978. I am therefor eligible for citizenship and and have the forms ready to fill in. I have been living in the UK for the past 8 years with on my NZ passport with my Right of Abode.
Questions:
1.12 - I am in a same sex relationship and my wife and I were married in NZ in on 19th August (we also had a civil union in NZ on 10th August). Do I tick "married" even though the UK marriage equality hasn't fully come in yet?
1.18 - How do I find the name of my local authority in relationship to the citizenship ceremony?
1.27 - I have no idea if I'm meant to put my Grandparents names and details. They were both British born. I have read the related information and can't work out whether I need to or not.
2.5 - I am self employed. Where do I find the address of tax office or tax office reference?
When I read what documents to send it it says my mothers full birth certificate (which I have); and either Her certificate of naturalisation or papers showing legal adoption or her expired passport. So I assume I need to get my mother to send an old passport over as the first two don't apply, what if she does not have an expired passport?
Lastly is there any way my passport and birth certificates can be checked instead of sending them in?
